   Take a clear photo of the front of each card and upload them. Only MediCall can see your files.

   Sign your IRS Form W-9

   You'll fill out and e-sign your W-9 on a secure form.

   Complete your background check & MVR

   Our screening partner will send you the disclosure and authorization to review and sign.
